Personal Home Care Services
At American Care Team, we offer assistance with activities of daily living, companionship, professional Skilled Nursing assessment, and treatment. We also offer medication administration to empower you to stay in your home. Our intake process follows below:
Initial Call From Client to Our Agency
During our initial conversation, the Office Manager will spend time with you to understand your current situation and the type of home care needs you require. When you have your first discussion we will need the following information to help develop a treatment plan to meet your needs (Please have ready prior to calling our agency):
Care Needs: We need to know about your level of need (wheelchair bound, walks independently, bed bound, needs assistance with toileting or feeding).
Hours Needed: How many hours do you need per week or per day for caregiver assistance.
Address: Your current address and phone number (this will determine if we can provide a local caregiver for your address).
Payment Type: Is it private paid, long-term care insurance, Medicaid, or Medicaid Waiver Program.
Medicaid or Medicaid Waiver Clients: We need your support planner’s phone number and agency name.
After the initial call is completed, you will receive a follow-up call from the Registered Nurse (RN). After the call, the RN will schedule a site visit to your home to develop a final care plan.
Home Care Services Provided
When the Registered Nurse (RN) has designed your treatment care plan and your payment type has been approved, your care can begin. Our caregivers are available to help you with all of the Activities of Daily Living:
Meal Preparation and Assistance with Eating: Our caregivers will assist in cooking meals and feeding when necessary. They can help with grocery shopping and they will oversee and monitor all of the dietary restrictions.
Personal Care and Hygiene: Our staff will help with bed-bath, getting dressed, showers, morning and evening care. Which includes skin care, brushing teeth, combing hair, or clipping nails (except for diabetic clients).
Toileting: Assistance with changing briefs and assistance in the restroom.
Walking Assistance, Transferring, and Positioning: Our caregivers will provide hands-on assistance during walking and transferring from your bed to your wheelchair or to your favorite chair at home.
Light Housekeeping and Errands: Our caregivers will clean only the patients surrounding area including their bathroom (not whole home cleaning). They will help with picking up mail from the mailbox, patient-related errands, trips to the pharmacy, and escorting to medical appointments in the patient-provided vehicle (caregivers cannot drive clients in their own personal vehicle).
Companionship: Our caregivers will spend quality time with our clients. They will listen and speak to the client and provide them with company.
Respite Care (Family Break from Care): Providing care for a loved one is rewarding. However, family members do need a break and that is where American Care Team comes in to assist with Short Term caring for your loved ones while you take an interrupted break.
